The Diabetes Support program is dedicated to providing holistic care for individuals managing diabetes. We focus on all aspects of diabetes management, including diet, exercise, and medication, ensuring that each client receives personalized support. Our team of professionals works closely with clients to develop effective management plans, aiming to enhance their quality of life and overall health. Whether you are dealing with Type 1, Type 2, or pre-diabetes, our program offers the resources and expertise needed to support your journey.

What do Diabetes Support Offer?
Our Diabetes Support program provides comprehensive management for all aspects of diabetes, including Type 1, Type 2, and pre-diabetes. We offer guidance on diet, exercise, medication, testing, and insulin adjustment, as well as strategies for prevention and pre-diabetes management. Our program is designed to support clients in managing their condition effectively and improving their overall health.
Schedule and Location
Appointments for diabetes support are scheduled for 30 to 60 minutes, depending on individual needs.
Clients can book appointments through our reception. We accept Medicare for all eligible services.
Eligibility Criteria
The program is available to PWHSAC clients and new members. It is open to individuals seeking assistance with diabetes management, whether newly diagnosed or needing ongoing support.
How to Register Your Interest
To register for diabetes support or get more information, you need a chronic disease management plan from your doctor or GP.
